Breakout Session Proposal Form

Colorado Council is using a new on-line registration system which will give you more flexibility and make your life easier! You can register more than one counselor at a time and pay by check or by credit card.  We will take payments at the registration table at the conference, but we ask you make every attempt to send payments beforehand to make registration flow more smoothly.

If you would like to pay by check, please continue through the on-line registration process and at the end you will be asked if you want to 1) pay now (for on-site registration only) 2) by check3) by electronic check or 4) by credit card. Click on the radio button for check, print your receipt and submit the bill to your purchasing department. Payment instructions are on the top of the receipt.  When your check is received, a payment confirmation e-mail will be sent to you.  Please be sure you ask your purchasing agent to include the copy of your registration with the check to ensure proper credit.

GRADUATE CREDIT:

There are 2 ways to gain credit for the Fall Update and December Conference of the Colorado Council on High School/College Relations:In-service/Professional Development Credit (No charge for this credit).5 credit is given for attendance at 1 day of the Fall Update.  At the end of the day, attendee is requested to fill out form with envelope attached.  Name, school info, address, and email is then entered into database and certificate is generated and mailed to attendee.  .5 credit is given for attendance at 1 day of the December Conference.  At the end of the day, attendee is requested to fill out form with envelope attached.  Name, school info, address, and email is then entered into database and certificate is generated and mailed to attendee.

Adams State Credit

1 college credit with Adams State College is given to attendees of 1 day of the Fall Update and 1 day of the December Conference OR 2 days of the December Conference.  At the end of the December conference*, attendee is requested to fill out Adams State College registration form, pay $50 (check or credit card) and take instructions on writing a 2-page reaction paper.  Registration forms and checks are mailed to Adams State.  When the papers are received and graded by instructor, a grade list is then mailed to Adams State which assigns the proper credit.  Transcript requests can be made to Adams State. *(Attendees who did attend 1 day of the Fall Update and 1 day of the December Conference may do this at the end of the first day of the conference.)
